Hours of Operation: 6:00 a.m. to 5:00 p.m. Monday through Friday Copies of Medical Records You may obtain copies of your previous medical records for your own personal use for $1.00 per page. Records requested on the day of service are given at no charge. In order to get copies of your medical records, you will need to fill out a Release of Information and present a valid photo ID. Please stop by Health Information Department, and we will be happy to assist you. Copies of your medical records that are sent directly to another physician or your insurance company are done so at no charge. |
